Text Message Opt-In

AgencyView (www.myagencyview.com) is software used by licensed health insurance agents. AgencyView sends text messages on behalf of an agent to that agent's existing clients: a link to a secure form about the client's own Medicare or health coverage, and at most one reminder if the form has not been completed. These are account notifications only. We never send marketing or promotional texts.

Clients opt in in one of two ways: on the secure intake form their agent sends them (shown below as clients see it), or verbally with their agent.

1. Opt-in on the intake form

The client opens the secure link from their agent. The last step of the form is Permission to Contact, shown below. The client chooses how they may be contacted (at least one way), types their name and agrees. Text messages are sent only if the client checks Text message. No box is pre-checked, and Text message is optional: a client can choose phone call or email instead and still submit the form.

How may Jane Smith contact you?

By providing your phone number and checking "Text message", you agree to receive SMS from AgencyView on behalf of Jane Smith of Smith Insurance Group, including links to secure forms and reminders about your Medicare and health insurance coverage. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not a condition of any purchase. Your mobile information will not be sold or shared with third parties for promotional or marketing purposes, and your text message opt-in data and consent will not be shared with any third parties. You also agree that Jane Smith of Smith Insurance Group may contact you about Medicare and health insurance options by the other methods you checked (phone call, email) at the phone number and email you provided, including calls sent with automated technology or prerecorded or artificial voice. You can withdraw your consent at any time by replying STOP to a text or telling Jane Smith. This permission applies only to Jane Smith, Smith Insurance Group and AgencyView sending on their behalf. 2. Verbal opt-in with their agent

By phone or in person, the agent reads this script and records the client's yes:

"Is it OK if I send you a secure form by text message and email so you can fill out your information? The texts come from AgencyView on my behalf, and you may get a reminder. Message frequency varies, and message and data rates may apply. You can reply STOP at any time to opt out, or HELP for help. Do I have your permission?"

Before AgencyView will send any text, the agent must check a box confirming they have each client's permission to contact them. This script is shown next to that box, and the confirmation is stored with the send. The first text to a client who opted in verbally is this confirmation:

AgencyView: Your form: www.myagencyview.com/intake/r/<link> You agreed to texts. Freq varies. Msg&data rates apply. HELP=help STOP=end
